Objective
What happened?
This should be confined to the facts – interpretation comes later. What did I see, touch, hear, smell, taste? This can include specific feedback that people gave. What did I do (the observable behaviour, rather than its impact or intention)?
Reflective
What emotions did I feel?
Interpretive
How can I interpret that?
What was really going on? What was good, helpful, effective or generated insights? What got in the way of progress?
Decisional
What should I do differently (or more of) next time?
